Health, posture and spinal x-ray assessment with the doctor. You will be given your x-rays and your personal biomechanics listing card to help enhance your therapist’s knowledge of your body for future massages and therapies. [~30 minutes]

Insurance Information

Lakeview Massage & Therapy does not offer direct billing to insurance companies, unless you are currently a patient of Upper cervical Family Chiropractic and Wellness and provide our therapists with the doctors note. Insurance from BCBS and HSA Accounts are possibly accepted. Client must be a current patient at Upper Cervical Family Chiropractic & Wellness and must have a note and diagnosis codes on file from the doctor.

We are more than happy to provide an itemized receipt for your session that you may submit to your insurance company for possible personal reimbursement. You are responsible for contacting your health insurance provider to verify that massage therapy is a covered plan benefit and for proper instructions on how to submit your claim. You are responsible for full payment at time of service.
